At Cartivous, your health and safety are our top priorities. In compliance with the United States Food and Drug Administration (FDA) regulations, CPAP and BiPAP equipment are classified as Class II Medical Devices. This means federal law requires a valid prescription for the purchase of certain respiratory products.

📝 What Makes a Prescription Valid?

To ensure quick processing of your order, please make sure your prescription includes the following information clearly written or typed:

• Patient Information: Your full name and date of birth.
• Physician Information: Doctor's name, credentials (MD, DO, NP, PA, DDS), contact information, and valid signature.
• Diagnosis: Usually written as Obstructive Sleep Apnea (OSA) or Sleep Apnea.
• Equipment Ordered: e.g., "CPAP Machine", "CPAP Mask", or "BiPAP".
• Pressure Settings: For machines, specific pressure settings (e.g., 10 cmH2O) or a pressure range for APAP machines (e.g., 5-15 cmH2O) must be included.
• Duration: Most prescriptions state "Lifetime", "99 Months", or "As Needed". If it has an expiration date, it must be currently active.

👨‍⚕️ Who Can Write The Prescription?

We accept prescriptions from a variety of licensed US medical professionals, including:

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
• Nurse Practitioner (N.P.) or Physician Assistant (P.A.)
• Dentist (D.D.S. or D.M.D.) - Often for CPAP or oral appliances
• Naturopathic Physician (N.D.) - If licensed to prescribe medical equipment in their state

Note: We cannot accept prescriptions from Chiropractors, Physical Therapists, or Psychologists.
